Insurance

Select providers (see above) do accept some types of insurance. However, in situations where insurance is not able to be used in office, Super Bills are provided in order for you to obtain reimbursement through your insurance company. Most insurance companies that cover mental health services will provide reimbursement for sessions fees when provided with a Super Bill. Please feel free to contact the office for any questions regarding this process.

 

Some important questions to ask your health insurance provider are: 

Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
How much does my plan cover for an out-of-network provider?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?

 

Payment

Cash, Debit Cards, all Major Credit Cards, Flexible Spending Accounts, and Health Spending accounts are accepted methods of payment.

Cancellation policy

 

Please contact the office directly for all cancellations and reschedules. Cancellations made less than 24 business hours in advance will result in a cancellation fee. 

Good Faith Estimate

Our rates range from $50-$350 per 50 minute therapy session depending on the therapist and the level of training and experience of the therapist. We do accept HSA or FSA (health or flex savings account) payment. 

 

Our goal is to connect you with the right therapist for you and recommend that you review the profile bio’s of each therapist and speak to the therapists or our administrative team to make sure you can make the best decision for your therapeutic needs.
   

Effective January 1, 2022 under Section 2799B-6 of the Public Health Service Act, a ruling went into effect called the "No Surprises Act" which requires practitioners to provide a "Good Faith Estimate" about out-of-network care. At Evolve Counseling & Behavioral Health Services, we work from a place of transparency regarding the cost of therapy sessions and the therapist and client collaborate together on length of treatment and overall care.

 

You have the right to receive a “Good Faith Estimate" explaining how much your care will cost.  

 

Under the law, if a patient requests a good faith estimate, health care providers need to give patients an estimate of the bill for services.

 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.

 

You can ask your provider for a Good Faith Estimate before you schedule a

service.

 

Make sure to save a copy or picture of your Good Faith Estimate.
