Evolve Counseling does not accept insurance in office, however Super Bills are provided for reimbursement through your insurance company. Most insurance companies that cover mental health services will provide reimbursement for sessions fees when provided with a Super Bill. Please feel free to contact the office for any questions regarding this process.

 

Some important questions to ask your health insurance provider are: 

Do I have mental health benefits?
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
How much does my plan cover for an out-of-network provider?
What is the coverage amount per therapy session?
Is approval required from my primary care physician?

 

Payment

Cash, Debit Cards, all Major Credit Cards, Flexible Spending Accounts, and Health Spending accounts are accepted methods of payment.

Cancellation policy

 

Please contact the office directly for all cancellations and reschedules. Cancellations made less than 24 hours in advance will result in a cancellation fee.
